These Terms & Conditions (“Terms”) govern your access to and use of millenniumdentist.com (the “Website”) and your interactions with our clinic and services (collectively, the “Services”). By visiting the Website, contacting us, or receiving care, you agree to these Terms.

HIPAA Notice: Our use and disclosure of Protected Health Information (PHI) is governed by our HIPAA Notice of Privacy Practices (NPP), which is separate from these Terms. You may request a copy at any time at our office or by contacting us.

2) Eligibility & Patient Relationship

Use of the Website is intended for individuals in the United States.

A patient-provider relationship is established only after you schedule, appear for care (in person or via approved telehealth), and we formally accept you as a patient. Viewing the Website or contacting us alone does not create a patient-provider relationship.

3) No Medical Advice on the Website

Information on the Website is for general educational purposes and is not a diagnosis or treatment plan. Do not rely on Website content for medical decisions. For emergencies, call 911 or go to the nearest emergency room.

4) Appointments, Cancellations & Missed Visits

Please arrive early and bring identification, insurance information, and relevant records.

Cancellation Policy: To avoid fees, cancel or reschedule at least 24 business hours before your appointment (or as otherwise stated in your appointment confirmation).

No-Show / Late Arrival: Missed or late arrivals may result in a fee and/or the need to reschedule.

5) Insurance, Billing & Payments

As a courtesy, we may verify benefits and submit claims; however, you are responsible for understanding your coverage, limitations, and out-of-pocket costs.

You agree to pay any patient-responsibility amounts (deductibles, co-pays, coinsurance, non-covered services, and any balances not paid by your insurer).

Payment is due at time of service unless otherwise arranged. We accept the payment methods stated by our office.

Interest/late fees may apply to overdue balances as permitted by law. Accounts may be referred to collections after reasonable notice.

6) Treatment Authorization & Informed Consent

By receiving Services, you consent to dental examination, diagnostics (including radiographs when appropriate), treatment recommendations, and reasonable variations or auxiliaries as clinically indicated. We will seek your informed consent before initiating treatment.

7) Prescriptions, Referrals & Post-Op Care

We may prescribe medications or provide referrals when clinically appropriate. Always follow instructions and ask questions if unclear. Contact us promptly with concerns, complications, or adverse reactions.

8) Telehealth (If Offered)

If telehealth is available, you consent to receive certain services via secure video/telephone. Telehealth has benefits and limitations, including potential technology issues. You agree to be located in a state where our provider is authorized during telehealth visits and to ensure a private, safe location for your session.

9) Communications: Email, SMS & Phone

By providing your email and/or mobile number, you consent to receive service communications (appointment confirmations/reminders, treatment coordination, billing notices).

Marketing messages (if any) require your consent and include an opt-out. You may unsubscribe via email link or reply STOP to SMS. Message/data rates may apply.

Electronic communications are convenient but may carry security risks. For sensitive matters, please use our designated secure channels.

10) Website Acceptable Use

You agree not to:

Violate any law or infringe rights of others;

Attempt to access non-public areas, interfere with Website operation, or introduce malware;

Use bots/scrapers without our prior written consent;

Misrepresent your identity or submit false information.

Your use of the Website is also subject to our Privacy Policy. PHI is governed by our HIPAA NPP. If there is any conflict regarding PHI, HIPAA and the NPP control.

No Guarantee of Outcomes: Dental outcomes depend on many factors, including individual health, compliance, and healing. We do not guarantee specific results.

15) Limitation of Liability

To the maximum extent permitted by law, Millennium Dental Clinic and its providers, employees, and agents are not liable for indirect, incidental, special, consequential, or punitive damages arising from your use of the Website or Services, even if advised of the possibility. Our total liability for any claim related to the Website will not exceed $100. These limitations do not apply where prohibited by law and do not limit our professional obligations to patients under applicable healthcare law.

16) Indemnification

You agree to indemnify and hold harmless Millennium Dental Clinic and our providers, employees, and agents from any claims, damages, liabilities, costs, or expenses (including reasonable attorneys’ fees) arising from your violation of these Terms or misuse of the Website.

19) Dispute Resolution (Optional Arbitration Clause)

At our election, disputes arising out of or relating to these Terms or the Website will be resolved by binding arbitration on an individual basis under the rules of the American Arbitration Association in King County, Washington. Class actions and class arbitrations are not permitted. You may opt out of arbitration by sending written notice within 30 days of first using the Website to the address in Section 1.
